Primary residential treatment centers function as an addict's first stop on the long road to recovery. Patients reside within the facility 24 hours a day, guided by regimented, personalized programs.

A full service treatment center provides initial patient assessment, supervised detoxification if necessary), and individual and group counseling under the direction of a full range of medical professionals, including physicians, psychiatrists, addiction counselors and therapists. They may provide specialty treatment programs such as anger management or cognitive behavioral therapy (CBT). Outpatient programs may be integrated with the residential treatment programs, but the primary focus is on the resident patients.

Primary residential treatment centers may be federally funded, private, non-profit, county- or state-run. Patients commonly stay around 30 days, and at the end of primary treatment, some facilities offer transition to halfway houses or extended care programs.
